MediaWiki Contribution Page: Ethical Documentation in Dementia Care
Welcome! This page is part of your activity for Module 3: Privacy Risks and Ethical Documentation. You will use this space to contribute your revised documentation sample, based on feedback from the CareWise Companion chatbot, and include a short reflection on the ethical choices you made.
How to Contribute
Step 1: Prepare Your Content
Before contributing, make sure you have:
- A revised documentation note that follows ethical and privacy best practices
- A short reflection (2 to 3 sentences) on what you changed and why it matters
Example Reflection Prompt: “I revised the note to remove identifying details and added language that better respects the resident’s dignity. This change helps promote privacy and supports ethical documentation in dementia care.”
Step 2: Add Your Contribution
- Log in to MediaWiki using your course account.
- Scroll to the bottom of this page and click on "Add Topic" or use the "Edit" button.
- Create a new heading using your initials and a title. For example:

- Paste your work under the heading:
- First, paste your revised documentation sample
- Below that, add your 2–3 sentence reflection
- Click “Save changes” when you're done.
