MediaWiki Contribution Page: Ethical Documentation in Dementia Care

Welcome! This page is part of your activity for Module 3: Privacy Risks and Ethical Documentation. You will use this space to contribute your revised documentation sample, based on feedback from the CareWise Companion chatbot, and include a short reflection on the ethical choices you made.


How to Contribute

Step 1: Prepare Your Content

Before contributing, make sure you have:

  • A revised documentation note that follows ethical and privacy best practices
  • A short reflection (2 to 3 sentences) on what you changed and why it matters

Example Reflection Prompt: “I revised the note to remove identifying details and added language that better respects the resident’s dignity. This change helps promote privacy and supports ethical documentation in dementia care.”


Step 2: Add Your Contribution

  1. Log in to MediaWiki using your course account.
  2. Scroll to the bottom of this page and click on "Add Topic" or use the "Edit" button.
  3. Create a new heading using your initials and a title. For example:
  == JP – Revised Documentation Note ==
  1. Paste your work under the heading:
    1. First, paste your revised documentation sample
    2. Below that, add your 2–3 sentence reflection
  2. Click “Save changes” when you're done.
