Complex Care Workflow Library – “A Sudden Shift”

This page is used in **Module 4: Digital Technology in Complex Systems** to explore how digital tools support crisis workflows in dementia care.

After watching the video **“A Sudden Shift (Sundown Syndrome Management)”**, add your own short analysis of the workflow and an SBAR-style digital message.

How to Contribute

  1. Scroll to the **Learner Entries** section.
  2. Copy the **Learner Template**.
  3. Paste it **below the template**, then replace the placeholder text with your own work.
  4. Use either your **first name + initial** or your **learner ID** as your section heading.
  5. Keep resident and workplace information **anonymous**. Do not use real names or identifiers.

What to Include

For your entry, you will:

  • **List 2–3 digital tools** used in the scenario
 (e.g., behaviour tracker, alerts, messaging app, documentation system).
  • **Map the workflow in 4–6 steps**, from the first sign of change to the team response.
  • Identify **one risk point** where a digital breakdown could create safety concerns.
  • Write a brief **SBAR-style message** that could be sent in a secure chat tool
 (Teams, Slack, WhatsApp, etc.).
  • Add **one sentence of reflection** on how digital tools helped support person-centred care.

Workflow steps
  1. Evening PSW notices Mr. K becoming increasingly restless and pacing near exits.
  2. PSW logs the behaviour in the electronic tracker; system flags “wandering / exit-seeking.”
  3. An alert is sent to the charge nurse’s device and the unit dashboard.
  4. Charge nurse reviews the history, messages the team in the secure chat, and assigns roles.
  5. Team adjusts the environment (lighting, noise) and offers a calming activity.
  6. Behaviour settles and follow-up is documented in the digital record.
Risk point
If the electronic alert failed or was missed, the team might not respond quickly,
 increasing the risk of exit-seeking and injury.
SBAR-style message for secure chat
S (Situation): Mr. K is increasingly restless and pacing toward the exit near the lounge.
B (Background): History of evening wandering and sundowning; similar episode last week.
A (Assessment): At rising risk for exit-seeking if we do not intervene soon.
R (Recommendation): Request support to redirect Mr. K to a calmer area and start his evening routine; please confirm who can assist at the door and with documentation.
Reflection
The digital tracker and alert helped the team respond quickly and coordinate tasks,
 reducing the risk of Mr. K leaving the unit while still supporting his dignity.
